Abstract

The integration of clinical pharmacists into healthcare teams has expanded the role of pharmacy beyond traditional medication dispensing. Clinical pharmacists contribute to medication review, identification of drug-related problems, dosage optimization, patient counseling, adherence improvement, and monitoring of therapeutic outcomes. This paper explores the contribution of clinical pharmacy services to rational medication use and patient-centered care. It examines the potential benefits of pharmacist involvement in reducing medication errors, preventing adverse drug events, and improving treatment outcomes. The study suggests that stronger collaboration between pharmacists, physicians, nurses, and patients can enhance the quality and safety of pharmacotherapy.
